Welcome. We are so glad to have your child in the Sterling school district. We will need the following items for your child’s health file:
-Birth certificate
-Physical form filled out by a physician (If your child is eight or under and entering a Kansas school for the first time)
-Immunization Card or record of immunization: All students must be up to date on immunizations or have a waiver signed.
-Health questionnaire form filled out
