| 1. Report is to be typed
2. Each member will write a narrative report of his/her service
project. In the report the member will introduce his/her project
and fully describe it so the project judge will understand the project
in terms of being: well planned, organized, and executed.
3. Include a section that evaluates your project. Include:
The inspiration for your idea.
Student's own idea or source
of idea.
Based on significant problem
or need in community, school, etc.
Describe how you planned
and prepared for the project.
Explain your goals for this
project.
A chronological record of each step of the activity,
with recommendations for repeating and/or
improving.
Describe the time and effort spent on the project.
Describe any obstacles and how you overcame them.
Describe how other benefited from your project.
Include some testimonials.
Describe the results of your project.
Did the service project meet its
original goals?
Was there significant, demonstrated
impact?
Were a significant number of people
helped by this activity or were a few helped greatly?
Will this activity continue as
a result of your involvement?
Include pictures a s arecord of your project.
If your project resulted in a product, attach a
copy of it to yor report.
What reflections do you have now that your project
is complete?
What new knowledge/skills did
you acquire?
What ne business/social relationships
did you establish?
Explain the importance of service
for you and those you benefited.
4. Write thank you notes to contributors or those who helped you
in your project.
Attach notes and adviser will
stamp and mail.
Checklist for Project Report
1. Two copies of typewritten report.
2. Pictures and copy of completed project if applicable.
3. Thank you notes with addressed envelopes.
