To participate in these additional services, we need a completed consent turned into the office. This is a terrific opportunity to get preventative dental services. Prairie Star will bill all insurances for additional services. Consent forms are available below and are also available in the office. Turn consent forms into the office by January 29 (HS) and February 6 (GS). Please send a note to the office if you do not want them to participate in the screenings.
